Application Package
A complete application package consists of four items:
1. Registration Form
This is the basic form required to begin the process of adoption and is used for opening a file and selecting a suitable social worker based on location.
2. Application Form
This is a two page document with background information on you and your family (if applicable). All documents and information collected remains confidential and is shared only with your social worker. If you have any questions about how to fill it out, please call us at 1-800-935-4237.
3. Letter of Agreement
This is the more technical legal contract that explains in detail what services you can expect from both The Adoption Centre and your social worker and your signature is required to confirm your understanding of the terms.
4. Consent To Disclosure of
Information
This is a one page document
which gives us permission to request information and documents from the
agency you were previously registered with or from the Ministry of Children
and Family Development. If you’ve not previously been registered with
either an adoption agency or the Ministry of Children and Family
Development, there is space on this form to indicate this as well.
5. Payment
Preferred payment format is by cheque and is made in two or three installments, depending on type of adoption.
For the exact amount of the initial installment, please refer to our fee schedules. Please note that expenses may vary significantly throughout the process, depending on the type of adoption you choose and various other factors such as legal fees, mileage etc.
